In Juliet's flashbacks, Harper introduces herself to Juliet as her therapist on the island. Harper seems hostile to Juliet right from the beginning. Shortly after meeting Goodwin, Juliet makes some comments to him about Harper and he reveals that she's his wife.Juliet and Goodwin have an affair and Harper tells Juliet to end it or Ben will hurt Goodwin.
In the present day, Harper comes to Juliet and tells her that Ben wants her to go to the Tempest after Charlotte and Daniel and kill them.
