Ray, wearing a white lab coat, comes to see Minkowski and inject him with something to calm him down. He asks how Desmond is feeling and shines a pin light in his eyes. - Episode 4x7, Ji Yeon
Ray gets Sayid and Desmond, apologizes for the delay, and says the captain will see them now. As they go to the deck, Sayid notices the helicopter is gone and asks where it went. Ray says that Frank went on an errand. Sayid presses him, but Ray says that he's a doctor and doesn't know where Frank went. - Episode 4x9, The Shape of Things to Come
Ray's body washes up on the beach and Daniel says that he was their doctor on the freighter. Ray's throat had been slit. When Daniel uses Morse code to ask the freighter people what happened to the doctor, a message comes back that says, "What are you talking about? The doctor is fine." - Episode 4x11, Cabin Fever
The doctor asks what happened to the injured man Keamy brings to the freighter, but Keamy doesn't really answer him.Later, Omar tells the doc that the Morse code message he received from the island said the doctor washed up on shore with his throat slit. "But I'm the doctor," Ray says.
When Frank refuses to fly Keamy to the island, Keamy slits Ray's throat and throws him overboard.
