Claire wakes in the night, screaming. While others take care of her, Ethan fills water bottles. The next day, while Hurley is going over the plane's manifest, he calls Ethan "Lance". Ethan says, "No, it's Ethan." He tells Hurley he is from Ontario.When Claire goes into labor in the jungle, Charlie sends Ethan to get Jack, but awhile later, after they discover it was false labor, Charlie and Claire walk towards the cave and Ethan steps out and says, "Well, hello there."

Hurley tells Jack that Ethan was not on the manifest. They later discover that Charlie and Claire are missing. Jack and Kate are searching through the jungle when they get separated. Ethan confronts Jack and says that if he doesn't stop following, Ethan will kill Charlie or Claire. Jack continues to follow and he and Kate soon come upon Charlie hanging by his neck from a tree.
