Juliet says he's lying when Jack tells Tom that Juliet wanted him to kill Ben. Tom kicks her out of the surgery, but goes to get her when Ben wakes and wants to talk to her. After she talks to Ben, she asks Jack to finish the surgery. She shoots Pickett so that Kate and Sawyer can escape to the main island.
