Juliet Shoots Danny

In flashbacks, Dr. Richard Alpert approaches Juliet about a job that involves something to do with women. He has her look at an X-ray of a womb and says that she can have all the resources she needs to help the woman. Juliet says that her ex would never allow her to take the job. The only way would be if he were hit by a bus.

Juliet is doing some sort of experiment to try and get her ill sister pregnant. Her over-bearing ex-husband, Edmund Burke, wants in on the experiment, but she wants to be left alone. When the experiment is successful she tells Ed and he wants to publish the experiment, but she doesn't want to. He is then hit by a bus.

Dr. Alpert, along with Ethan Rom, asks her to take the job. She wants to take her sister, but he says her sister wouldn't get the treatment she needs. The job is not quite in Portland.

On the island, Kate and Sawyer escape, but are pursued by Pickett, Jason, and another man. Alex helps them to escape, but then asks them to help her free Karl.

Meanwhile, Ben wakes up from his surgery and asks to talk to Juliet. After speaking with Ben, Juliet asks Jack to finish the surgery.

Juliet shoots Pickett to allow Kate, Sawyer, and Karl to get away, but says that Alex must stay. Later, after Jack gets the tumor out of Ben and Ben seems to be doing okay, Juliet tells Jack that Ben said if she made sure he lived, he would allow her to go home.
