Jack says they can't get into the hatch and suggests that they go back to the caves and tell everyone that everything will okay. Locke comes back to the hatch and when Kate follows, Jack decides to follow too.
Jack arrives at the hatch to find Kate and Locke gone and climbs down. Desmond is there with a gun to Locke's head.
Meanwhile, Shannon looks for Vincent in the jungle and thinks she sees a dripping wet Walt saying something she can't understand.
In flashbacks, Jack has to decide whether to help a man, Adam Rutherford, or a woman, Sarah, who have been brought in from an accident and he decides to help the woman.
The man ends up dying. Jack promises the woman he will fix her, but then when he can't do it, he is upset and goes running in a stadium. He meets Desmond in the stadium, who tells Jack that maybe he did fix the woman. Jack goes back to the hospital to find out that Sarah won't be paralyzed after all.
