Why All This?
Jack works on Ben while Tom stands nearby looking faint. He says he doesn't like blood too much.Jack says, "So, if you really can get off the island, why didn't you take him to a facility, why all this?"
"Because," Tom says, "after the sky turned purple--"
Blood spurts out of Ben's back and Jack steps back. A monitor beeps.
"What the hell happened?" Tom wants to know and Jack says he nicked an artery. Tom says, "Isn't that what you already did?"
"Yeah, well," Jack says, "that was on purpose." Tom wants to know if he can fix it.
The Bosses Daughter
Sawyer, Kate, Alex, and a really out-of-it Karl make it to the boat.As they get the boat ready, Sawyer says to Alex, "So, you're the bosses daughter, huh? I never saw that coming."
He runs back to Karl and says, "Let's go, Cheech, we're going for a little ride." They get Karl into the boat and he looks at Sawyer and says, "Danny."
"No, Sawyer," Sawyer says.
Karl says, "Danny," again and Sawyer turns to see Danny with a gun pointed at him. Sawyer begins walking toward him and Kate yells no and runs towards Sawyer.
Juliet calls Danny's name and he points the gun at her, but she shoots him before he can shoot her.
Alex Left Behind
Jack says there is too much blood and he can't see. He needs Tom's help.On the beach, Juliet walks up to Danny and tells Sawyer to go. She says that Alex has to stay. Alex asks why and Juliet says, "We both know your father will never let Karl live if you go."
Alex looks devastated and Juliet says she's sorry. Alex says goodbye to Karl. Karl says he loves her.
They start to push the boat out and Juliet tells them to wait. She pulls the walkie-talkie from her waist band.
In the operating room, Kate comes over the walkie and asks if Jack's there. Jack tells Tom to hold the walkie up to him.
Kate tells him that the blond woman is letting her go. He asks if she and Sawyer are safe and Kate says yes. He asks her to tell him the story.
She tells the story of him doing surgery on the girl and he messed up. Sawyer and Juliet see how much she cares for Jack as she is telling the story. Jack gets teary-eyed as well.
He continues to work on Ben and gets a steady heart beat.
Jack says, "I need you to make me a promise, Kate. Promise me that you'll never come back here for me." She wants to know where he is, but Jack tells Tom to turn it off. Kate cries and they get in the boat.
Alex and Juliet watch them sail off.
Juliet's Flashback - Not Quite in Portland
A morgue employee covers Edmund's body in the morgue and tells Juliet that they have some paperwork for her to sign. She goes to sign the documents and begins to cry.Why Did Juliet Want Jack to Save Ben?
Juliet joins Jack in the observation room. She asks if he was able to remove the tumor and he says yeah and that she should do a biopsy to see if it's malignant. She nods.He asks what happens now, if he just goes back to his cell. She says until they figure out what to do with him.
He wants to know what Ben said to make Juliet want to save his life. She says it doesn't matter, but he says it matters to him and after everything that he's been through, he wants to know.
Juliet's Flashback - Not Quite Portland
As she cries someone gives her a tissue. She looks up to see Ethan and she asks if they've met."Hello, Dr. Burke," Dr. Alpert steps into the room.
Dr. Alpert gives his condolences. She asks what he's doing there and he says he saw on the news what happened and that her office said she was at the morgue. He introduces Ethan as one of his colleagues.
Ethan says it's a pleasure and seems genuinely happy to meet her.
"He was hit by a bus," Juliet says.
Dr. Alpert says he knows, they said on the news. Juliet says, "No, no, in our interview I said that I wanted him to get hit by a bus."
He says she can't blame herself and he doesn't even remember her saying that.
She asks again why he's here. He knows the timing is bad, but they are serious about having her work for them. He wants her to give them six months and she'd be back by the time her sister gives birth.
"How did you know that my sister--"
"Well we're very thorough in our recruitment process."
Juliet wants to know if her sister can come and he says no, that it's pretty remote and she wouldn't have access to the treatment she needs. Juliet points out that there are plenty of clinics in Portland, but Dr. Alpert says, "Well, we're not quite in Portland."
